Daiana Mendez

Young Mothers Program and Permanent Supportive Housing Program

From a homeless teen facing abuse and addiction, Daiana Mendez transformed her life and now leads others toward healing as a counselor and advocate.

Daiana Mendez

Born and raised in Spanish Harlem, Daiana Mendez’s life took a dramatic turn when she became pregnant at just 14. Forced out of her home and onto the streets, she endured homelessness, substance use, and domestic violence at a young age. Daiana was accepted into the Young Mothers Program (YMP) two months before turning 16, becoming the youngest participant ever.

At YMP, she found structure, safety, and a path to reclaim her life and her son. Daiana reunited with her baby, completed high school, and entered permanent supportive housing. Over time, she built a career in substance use counseling, earning her CASAC license and rising through the ranks to become an Assistant Director at the healthcare services provider where she works. She now guides others through their darkest moments.

Daiana is a proud mother of four and a determined student working toward a graduate degree in psychology. Her dream is to one day open a one-stop nonprofit for women and children in need. From a teenager locked in an abusive home to a woman leading others toward healing, Daiana embodies the strength of recovery and the power of second chances.
